Data processing related to examinations, diagnoses, treatments, hospital procedures, and staff processes is the subject of separate notices that will be provided specific to each activity.
Your information is collected electronically when you fill out the fields on the website and submit the form. Any additional information you provide while your request is being processed is also used for the same purpose.
The forms do not require you to upload files or photos. Please do not include medical reports, ID numbers, or information about third parties in the inquiry form. If you need to share your medical history, please use the appropriate section in the preliminary interview.
The provision of this privacy notice does not constitute the granting of explicit consent. The declaration of consent for the preliminary consultation is explained in a separate explicit consent form. Permission for marketing or advertising is not the subject of this form.
Your request may be viewed by authorized physicians and the communications team to the extent required by the nature of the work. Information may be transferred—limited to the relevant purpose—to hosting and technical support service providers for the receipt and storage of the request, to the email service provider for the transmission of the notification, and to authorized public institutions and judicial authorities in the event of a legal obligation.
The email notification includes all responses from the form. When Turnstile is used, a security token is sent to Cloudflare for bot verification; the health responses from the form are not included in the Turnstile verification request. The Cloudflare service may also process technical connection information from the browser.
The use of overseas infrastructure for email, hosting, or third-party services may result in the transfer of data abroad. The legal basis for such a transfer is determined in accordance with Article 9 of the Law, based on the specific service used and the nature of the data flow. Explicit consent obtained during a preliminary consultation does not serve as a general authorization for all transfers abroad.
Communication and preliminary interview form records are retained for as long as necessary to track the request. The default automatic deletion setting for the form system is 90 days. The deletion of expired records is carried out as part of the daily cleanup process; this period should not be confused with the retention periods for records that must be retained separately to comply with legal obligations or to protect rights.
Deleting a record from the database does not automatically delete the notification email or the backup copy. These copies are managed separately, taking into account their purpose, access requirements, and applicable retention obligations. Information for which the retention requirement has expired is deleted, destroyed, or anonymized using appropriate methods.
Under Article 11 of the Law, you have the right to learn whether your personal data is being processed, to request information, to learn the purpose of processing and the recipients to whom it has been transferred, to request correction or, if the conditions are met, erasure/destruction, request that these actions be communicated to the relevant recipients, object to a decision made solely through automated processing that is detrimental to you, and request compensation for damages resulting from unlawful processing.
