The inquiry form is used to submit your general questions. The preliminary interview form, on the other hand, collects your contact information and the medical history you choose to share in separate steps. Please fill out only the required fields; do not submit information about other people or any unwanted personal identification or document details.
Submitted requests are stored in the WordPress database, and an email notification is generated so that the authorized team can track them. Even if the email fails to send, the request record is retained. The database cleanup period during initial setup is 90 days; the retention of emails and backup copies is managed separately.
Access to form submissions is restricted to authorized administrator accounts. Security authentication, request throttling, and server-side field validations are implemented to minimize abuse. These measures should not be interpreted as a guarantee of absolute security on the Internet.

The location of the hosting and email infrastructure, as well as data flows from third-party providers, are taken into account when assessing cross-border data transfers. A general consent for health data does not constitute authorization covering all of these transfer processes.
